Insurance & payment

Know your coverage before you start.

Cost shouldn't be a mystery. Send us your insurance details and our admissions team will check your benefits and explain what's covered, before you commit to anything.

How it works

Four steps, no commitment.

Checking your coverage is free and doesn't sign you up for anything.

  1. Send your details

Use the form below or call us. We only need your name, how to reach you and your insurance company to get started.

  2. We check your benefits

Our team contacts your plan to confirm your behavioral health and substance use disorder benefits.

  3. We explain it plainly

You'll hear what's covered, any costs you may be responsible for, and whether your plan works with Revive.

  4. You decide

If it's a fit, we schedule your start. If not, we'll point you toward other options and resources.

Ways to pay

Coverage we can review with you.

Every plan is different. Whatever kind of coverage you have, or don't have, start with a conversation.

Washington Apple Health (Medicaid)

Washington's Medicaid program includes benefits for substance use disorder treatment for eligible members. Tell us your Apple Health plan and we'll confirm coverage.

Employer and private insurance

Most health plans include benefits for substance use disorder and mental health treatment. We'll check your specific plan, your deductible and whether we're in network.

  • Have your insurance card handy when we talk.
  • Some plans need a pre-authorization. We'll tell you if yours does.

Marketplace plans

Plans bought through Washington Healthplanfinder include behavioral health benefits. We'll review what your plan covers for treatment at Revive.

No insurance, or not sure

Being uninsured or unsure of your coverage isn't a reason to wait. Call us and we'll talk through your options, including help applying for coverage you may qualify for.

  • Many people qualify for Apple Health and don't know it.
  • We'll explain any self-pay costs clearly and up front.
We check before you start.

Plan networks change, so our admissions team checks your specific plan and explains any expected costs before your first session. A benefits check isn't a guarantee of coverage or payment. Your insurance plan makes the final decision on what it pays.

Free coverage check

Have your benefits checked.

Send this short form and we'll call you with what we find. It helps to have these ready when we talk:

  • Your insurance card (front and back)
  • The policyholder's name and date of birth
  • Your member or subscriber ID
  • Any referral or authorization letters

For your privacy, this form doesn't ask for your member ID or medical details. We'll collect those securely by phone.

Check my coverage

Tell us what coverage you have. We'll check your benefits for treatment at Revive and call you during business hours with what we find. A benefits check isn't a guarantee of coverage or payment; your plan makes the final decision.

Fields marked * are required. This form isn't monitored for emergencies. In a crisis, call or text 988, or call 911.

Coverage questions

Common insurance questions.

See all questions
Does health insurance cover addiction treatment?

Most health plans include benefits for substance use disorder treatment. Federal parity law generally requires those benefits to be no more restrictive than a plan's medical benefits. What you pay depends on your specific plan, which is why we check before you start.

Does a benefits check guarantee my plan will pay?

No. A benefits check tells us what your plan reports about your coverage at that time. Your plan makes the final decision on claims, and benefits can change. We'll explain what we find and what it may mean for you.

Will checking my coverage cost anything?

No. Checking your benefits is free and doesn't commit you to treatment.

What if my plan isn't accepted?

We'll tell you plainly, talk through any other options, and help you find care that works with your coverage if Revive isn't the right fit.

Is my information kept private?

Federal law gives records of substance use treatment extra privacy protection. We use what you share on this form to check your coverage and contact you. Read our website privacy notice for how the form works.
