Website privacy notice
How this website handles the information you share through its forms, and what you can do if you'd rather not use them.
Last updated: October 11, 2026
What this notice covers
This notice covers the Revive Behavioral Health website only: the pages you browse and the forms you can send from it. It doesn't replace the privacy information you receive when you begin care with us. If you have questions about how your health records are handled once you're in care, ask our admissions team.
You don't have to use the forms
Every form on this site is optional. You can call us at (206) 746-3500 during business hours (Mon–Fri 9am–5pm · Sat 9am–2pm) instead. If you're in crisis, call or text 988. In an emergency, call 911. Our forms are not monitored for emergencies.
What the forms collect
We ask only for what we need to contact you and point you to the right next step. Please don't include medical details, diagnoses or medication information in a form. We'll talk with you privately by phone.
| Form | What it asks for |
|---|---|
| Request a call back (Get Help Today page) | Your name and phone number; optionally your email, who you're reaching out for, the kind of help you're looking for, voicemail preference and a short practical note. |
| Insurance coverage check | Your name and phone number, type of coverage; optionally your email, insurance company name, best time to call and voicemail preference. It does not ask for member ID numbers. We collect those by phone. |
| Professional referral | The referrer's name, organization and phone; optionally email. For the client: initials only, a general release or discharge timeframe and the main need. No full names, dates of birth or records. |
| Contact message | Your name, email and message; optionally phone number and topic. |
The choices you select, such as the kind of help you're looking for, can show that you or someone you know may be seeking substance use or mental health care. We treat everything you send as sensitive.
How your information is sent and used
- Forms are sent over an encrypted (HTTPS) connection to the secure form service Revive Behavioral Health uses to receive website requests.
- We use what you send to respond to your request: to call you back, check your coverage, coordinate a referral or answer your message.
- We don't sell the information you send, and we don't use it for advertising.
- Form contents are never placed in the page address (URL), and this site doesn't record what you type.
Cookies, analytics and other services
- This website does not set cookies and does not use analytics, advertising or tracking tools.
- Fonts and most images are served directly from this website.
- The video and some photos on the homepage and Services page are licensed stock media delivered by Pexels. When those load, Pexels' servers receive standard technical information such as your IP address and browser type. People shown in that media are models, not Revive Behavioral Health clients.
- Map and "Get directions" links open Google Maps only when you choose to click them.
- Links to other organizations on our Resources page take you to their websites, which have their own privacy practices.
Email, phone and fax
